Service Cancellation Request

Please complete the form below to initiate your service termination request. Upon completion, we will be in touch with next steps. The cancellation form must be completed by an Account Administrator, and submitted 30 days prior to service termination.

Disconnect date must be at least 30 days in advance.

Provide the email address you’d like the shipping label sent to.
Enter the address from which your Sonic-owned equipment will be shipped.

Cancellation Policy

  • All requests for termination must be submitted by a current authorized administrator on the Customer’s Sonic account. If a request is submitted by someone other than an authorized administrator, the request will not be processed and all services will remain active and billed.
  • All requests for termination must be submitted 30 days prior to the requested disconnect date.
  • The contract term for the service(s) being disconnected must expire on or before the requested disconnect date. If the service is still in term, a Sonic representative will be in contact to discuss the early termination fee.
  • Early termination fees are 100% of the remaining balance left in the term, plus any waived setup, equipment or construction charges as well as any promotional credits and/or incentives given upon contract execution.
  • A final bill will be emailed upon termination. If the account is currently set up on automatic billing to a credit card, all final charges and applicable early termination penalties will be automatically billed to the credit card on file. All service(s) will continue to be billed until the disconnect is complete whether the circuit is in use by the Customer or not.
  • Customer must return any Sonic-provided equipment within 5 business days of the effective disconnect date. A return shipping label will be emailed to the Customer by Sonic’s Network Operations Center when we begin to process the disconnect. If all equipment is not returned within 5 business days, all unreturned equipment will be billed in full on the Customer’s final bill.
